Caregiver Connections™ 

Rockford’s Caregiver Support Program 


 

Are you an at-home caregiver who is caring for a loved one with Alzheimer’s or another type of dementia and need someone to lean on?

 

The Caregiver Connections Program is comprised of both family and professional caregivers who have volunteered to provide support for other caregivers in our community.

 As caregivers who have been in your shoes and have faced the same struggles you have, they understand what you are going through and can provide the support you may need.

 This unique program partners you with a personal “Caregiver Buddy” to help you in your time of need and offers:

  • education
  • support
  • guidance
  • advice

or just a friendly ear to talk to.
